Faith Community Partnerships

In Southeast Michigan, Trinity Health Ann Arbor and Chelsea Hospital seek to improve the health of the communities we serve. One of the many ways we do that is by building relationships with local faith-based organizations and congregations from all faith traditions through a program called Faith Community Partnerships.

The Faith Community Partnership Program is a network of individuals and faith communities with interest in promoting whole health: mind, body, and spirit within their congregations. This program partners with faith communities active in health ministry, along with registered nurses, lay leaders, faith leaders and congregants to integrate faith and health in their communities.

Trinity Health Faith Community Nurse Liaisons assist and empower faith communities to address the unique health needs of their congregations by providing tools and information to supplement your health promotion and health education endeavors. Education and networking opportunities are also provided.
