Who Is Diabetes Prevention For?

This program is for people who have prediabetes. Although prediabetes might not make you feel sick, you face a higher risk of future health issues.

If you end up with a Type 2 diabetes diagnosis, you also have a higher likelihood of experiencing:

  • Chronic kidney disease
  • Hearing issues
  • Heart disease, including high blood pressure, which can lead to stroke
  • Nerve damage that can cause foot problems and increase your risk of amputation
  • Vision loss

Type 2 diabetes prevention includes lifestyle changes that bring blood sugar levels back to a healthy range. These changes don’t come easily, which is why Trinity Health Michigan gives you the information and support you need to succeed.

Diabetes Prevention at Trinity Health Michigan: Why Choose Us?

Our Diabetes Prevention Program is a national, research-based program that prevents or delays the onset of Type 2 diabetes. An experienced lifestyle coach helps you make and sustain changes that enable many people to steer clear of a diabetes diagnosis.

Who's Eligible?

The program is open to non-pregnant adults age 18 years and older who:

  • Have a BMI equal to or greater than 25 (23 or over if Asian decent)
  • Do not have a diagnosis of type 1 or type 2 diabetes
  • Have a blood test within the past year showing: Hemoglobin A1C: 5.7 to 6.4% OR Fasting plasma glucose:100–125 mg/dL
  • Have a diagnosis of pre-diabetes or a history of gestational diabetes

Type 2 Diabetes Prevention: What to Expect

Two of the most impactful steps in lowering blood sugar levels and preventing a Type 2 diabetes diagnosis are:

  • Losing weight
  • Increasing physical activity levels

These goals can be challenging to meet, but you can do it. Specialists at Trinity Health Michigan show you how and provide you with support to help you achieve excellent results. You learn how to make healthy habits part of your daily life, lowering your long-term risk of Type 2 diabetes.

Our Diabetes Prevention Program lasts one year. It involves:

  • Attending 16 weeks of classes
  • Going to maintenance sessions for ongoing support
  • Managing stress
  • Learning more about healthy eating and becoming more physically active
  • Tracking your weight, eating habits and physical activity
  • Problem-solving, such as finding healthy food options when eating out
