ST. JOSEPH MERCY PRESENTS OFF CAMPUS DIABETES EDUCATION CLASSES

Port Huron, MI… In an effort to provide better access of diabetes education to residents across St. Clair County, the Mercy Diabetes Center at St. Joseph Mercy Hospital is offering two off campus classes in February and March.

The classes, offered in Fort Gratiot and Imlay City, provide in depth Diabetes Self-Management Training. Certified by the Michigan Department of Community Health, and awarded recognition from the American Association of Diabetes Educators (AADE), this series of classes is designed to meet individual needs, whether attendees are newly diagnosed or would like to update their self-care education program and skills.

Program topics include: Diabetes overview; nutrition management; medications; blood glucose monitoring and use of results; complications and risk factors; lifestyle changes; exercise and activities; stress and psycho-social adjustment; family involvement and social support; preconception and pregnancy diabetes management; and gestational diabetes. The goal of the program is to educate and train attendees and their families in the self-management skills of diabetes; therefore a support person is welcome to attend.

Each class is a two day session held from noon to 4:30 p.m. On February 25 and 26, 2014 the class will be held at Mercy Health Center, 4190 24th Avenue, Fort Gratiot. On March 3 and 6 the class will be held at the Imlay City Senior Center, 395 E. Third Street, Imlay City. Registration is required, and you must have a referral from your physician to participate.

There is a fee for the Diabetes Self-Management Training Program. Medicaid, Medicare and many commercial and private insurance carriers reimburse program costs for ADA-recognized programs. Guidelines for reimbursement require a physician order. Educational scholarships are available for patients with insurance carriers that do not cover the cost.

To register to attend and to find out more about the course call 810-985-1840. For more information about the programs and services of St. Joseph Mercy Port Huron, visit online at mymercy.us; or follow us on facebook and twitter.
