How to Obtain Medical Records
You can request a copy of medical records for
- yourself
- your child's (under the age of 18)
- an individual whom you are their legal guardian*
If you are requesting a copy to be sent to yourself please complete the Request for Access to Health Information Form.
If you are requesting a copy to be sent to someone other than yourself, please complete the General Authorization for Use or Disclosure of Health Information.
*If you are requesting medical records for an individual whom you are their legal guardian, please provide copies of guardianship papers along with the appropriate request form and mail to:

Birth Certificates and Death Certificates
Birth and Death certificates can be obtained from the Washtenaw County Clerk's Office.
Visiting us at St. Joseph Mercy Ann Arbor
Park in Visitor Lot Q and enter the hospital via the Family Birth Center/Imaging Center
